PRESCRIPTIONS When it is time for your prescription renewals, please call us before 2PM to allow adequate processing time. Our staff will ask you for the following information:
If you are experiencing any side effects or allergic reactions from the medications, please make our staff aware of that immediately. REFERRALS We provide complete primary care to all infants, children, and teenagers. We encourage patients to visit us for any problems. If we find that your problem is outside of our area of practice, we will gladly refer you to a specialist trained to treat your specific problem. As required by insurance companies, in order to obtain a referral, the patient must be first seen in our office. Please check with your insurance for any detailed questions on referrals. TEST RESULTS Our office does not routinely notify patients with normal laboratory or diagnostic test results. We will notify you by phone or mail regarding any results which require further evaluation. PATIENT EDUCATION If you would like any educational handouts or pamphlets, please ask our staff. We will be glad to provide you with additional information. Also, please ask our medical staff if you have any questions in regards to your diagnosis, treatment, or prescriptions. INSURANCE & BILLING We accept many insurances for our patient’s convenience. Please bring all insurance information, e.g. cards, with you on your first visit. We accept cash, personal checks, or certain credit cards at time of service. Please notify us of any changes in your address, phone number, or insurance carrier. |
